It's easy to register to walk as an individual or a team!
To register, please provide the information requested below.

Registration

You may register online until April 29, 2009. There is no charge to register for the walk if your registration form is received before then.

After April 29 there is a $15 registration fee, and online registration is no longer available. Instead, please select one of the buttons below to open the correct form and print it. Then complete and mail the form with your $15 check or money order (payable to Hospice of Michigan) to:

WAIVER
I hereby waive and release any and all rights and claims, known and unknown, for damages against Hospice of Michigan, the City of East Grand Rapids, as well as any other persons or groups connected with this event for any and all injuries or damages which I may suffer while taking part in this event. I am not aware of any physical condition which could prohibit my participation.
