To register, please provide the information requested below.

Registration

You may register online until May 1, 2008. There is no charge to register for the walk if your registration form is received before then.

After May 1, there is a $15 registration fee, and online registration is no longer available. Instead, please print out this form, complete and mail with your $15 check or money order (payable to Hospice of Michigan) to:

I hereby waive and release any and all rights and claims, known and unknown, for damages against Hospice of Michigan, the City of East Grand Rapids, as well as any other persons or groups connected with this event for any and all injuries or damages which I may suffer while taking part in this event. I am not aware of any physical condition which could prohibit my participation.
