Purchasing a memorial brick to place in the courtyard of Hospice of Michigan's Corporate Headquarters is a unique and special way to honor someone, or memorialize a loved one.

Donors interested in purchasing a brick or stone should print and complete this form.

There will be an annual installation of engraved pieces each summer. Notification will be sent to donors and designated individuals when the pieces have been installed.
